About The Position
CarePartners is seeking a Nurse Specialized in Wound, Ostomy & Continence (NSWOC) to provide consultation for patients in clinic and community settings. This role is ideal for individuals who enjoy caring for a diverse range of clients with various wound etiologies and care modalities. The NSWOC will utilize their expertise in wound and ostomy care to promote patient self-management, lead interdisciplinary teams, and contribute to regional wound care guidance. CarePartners offers comprehensive orientation, ongoing clinical support, and necessary equipment, including an iPad. The position offers significant schedule flexibility and is open to Full Time employment in the St. Catharines and Niagara area.
Requirements
- Registered Nurse (RN) degree
- Current College of Nurses of Ontario (CNO) registration in good standing
- Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOC, formerly CAET), CNA exam may be pending
- Several years of escalating experience managing wounds and ostomies
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver’s license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- A current vulnerable sector check and a clear background check

Responsibilities
- Provide wound and ostomy consultation for patients in clinic and community settings.
- Care for patients with consuming wounds, ostomy, and continence issues.
- Provide lower leg assessment including ABPIs.
- Critically appraise wound care literature and apply research evidence to practice.
- Provide classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomes.
- Practice conservative sharp wound debridement (CSWD) and use of other debridement methods.
- Provide edema management including bandaging based on complete lower leg assessment.
- Monitor criteria for adjunctive therapy for complex or recalcitrant wounds.
- Support patient safety by practicing according to policy and standards.
- Perform a thorough holistic assessment of each client and identify client needs using information from the client, caregiver(s), home environment, Ontario Health atHome referral, and other sources.
- Function as a member of a nursing team accountable for the nursing care it provides.
- Develop appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the Ontario Health atHome goal for treatment.
