Wound Care Specialist- Nurse Specializing in Wound, Ostomy and Continence- IIWCC, NSWOC, WCS (HNHB)
About The Position
CarePartners is seeking a Nurse Specialized in Wound, Ostomy & Continence (NSWOC) to provide consultation for patients in clinic and community settings. This role is ideal for individuals who enjoy caring for a diverse range of clients with various wound etiologies and care modalities. The NSWOC will utilize their expertise in wound and ostomy care, promote patient self-management, and collaborate with interdisciplinary teams to achieve optimal patient outcomes. The position involves contributing to regional wound care guidance and will include comprehensive orientation, ongoing clinical support, and necessary equipment such as an iPad. This is a full-time position open in the Simcoe/Delhi area, offering significant schedule flexibility.
Requirements
- Registered Nurse (RN) degree
- Current College of Nurses of Ontario (CNO) registration in good standing
- Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOC, formerly CAET), CNA exam may be pending
- IIWCC or WCS Certified - can be considered possibly different pay rate range
- Several years of escalating experience managing wounds and ostomies
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver’s license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- A current vulnerable sector check and a clear background check
Responsibilities
- Provide wound and ostomy consultation for patients in clinic and community settings.
- Care for patients with consuming wounds, ostomy, and continence issues.
- Provide lower leg assessment including ABPIs.
- Critically appraise wound care literature and apply research evidence to practice.
- Provide classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomes.
- Practice conservative sharp wound debridement (CSWD) and use of other debridement methods.
- Provide edema management including bandaging based on complete lower leg assessment.
- Monitor criteria for adjunctive therapy for complex or recalcitrant wounds.
- Support patient safety by practicing according to policy and standards.
- Perform thorough holistic assessments of each client and identify client needs using information from the client, caregiver(s), home environment, Ontario Health atHome referral, and other sources.
- Function as a member of a nursing team accountable for the nursing care it provides.
- Develop appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the Ontario Health atHome goal for treatment.
- Promote self-management by patients.
- Lead the interdisciplinary team to achieve optimal patient outcomes.
- Contribute expertise to individual practitioners, committees, and organizations guiding wound care in the region.
