About The Position

The Wound Care Registered Nurse (RN) or LPN/LVN works with nursing staff to assess and treat patients with complex wounds, ostomies, and/or continence conditions and to create care plans. The role provides consultation and direct assistance in managing altered skin integrity, including pressure necrosis, wounds, and fistulas. It also supports staff knowledge and competence in wound care through in-service programs and collects, analyzes, and documents wound-related information and statistics. The position includes clinical evaluations for potential wound admissions. This is a full-time position with an on-call rotation.

Requirements

Must be a licensed RN or LPN/LVN in good standing with the state, as required.
1–2 years of wound care experience required.
Reliable transportation required.

Nice To Haves

Wound Care Certification (WOCN) preferred.
1+ years of long-term care experience preferred.

Responsibilities

Provide consultation and direct assistance in managing patients with altered skin integrity, including pressure necrosis, wounds, and fistulas.
Offer in-service programs as needed to help nursing, rehabilitation, and medical staff maintain wound-care knowledge and competence.
Collect, analyze, and document information and statistics related to wounds in the facility.
Perform clinical evaluations for potential wound admissions.
