About The Position

Concordia Visiting Nurses is seeking a compassionate and experienced Wound Care Registered Nurse (RN) to join our growing home health team. This position is ideal for an RN who has a strong clinical background in wound management and enjoys providing individualized care in the home setting. As a Wound Care RN, you will have the opportunity to use your clinical expertise while helping patients heal, maintain their independence, and achieve the best possible outcomes.

Requirements

• Previous wound care experience strongly preferred
• Home health experience preferred, but we are willing to consider strong acute care candidates with significant wound care experience
• Strong assessment and clinical decision-making skills
• Ability to work independently and manage your daily schedule
• Comfortable providing care in patients' homes throughout the assigned service area
• Reliable transportation and the ability to travel between patient visits

Responsibilities

• Provide skilled nursing care to patients in their homes with a focus on wound assessment and management
• Perform comprehensive wound assessments and develop individualized plans of care
• Provide treatment for a variety of acute and chronic wounds
• Manage complex wounds, including pressure injuries, surgical wounds, diabetic wounds, venous ulcers, and other skin integrity concerns
• Perform wound measurements, documentation, and ongoing reassessments
• Educate patients and caregivers on wound care, prevention, nutrition, medications, and disease management
• Collaborate with physicians, therapists, home health aides, and other members of the interdisciplinary care team
• Recommend appropriate wound care products and treatment approaches based on the patient's needs
• Monitor patient progress and make appropriate changes to the plan of care
• Complete accurate and timely clinical documentation
• Help patients and families understand their care and feel confident managing their health at home
