About The Position

Bloom Healthcare is seeking a Nurse Practitioner or Physician Assistant to join their Wound Care Team. This role focuses on providing comprehensive wound care to home-bound patients in assisted living communities and private residences. The provider will conduct thorough assessments, develop evidence-based treatment plans, and tailor health management plans to individual patient goals. The position emphasizes building strong interpersonal relationships with patients in their homes and collaborating with a dedicated care team. Requirements

• Master's degree required
• Licensure in the state of Colorado
• Nurse Practitioner or Physician Assistant

Nice To Haves

• Wound certification preferred

Responsibilities

• Deliver wound care at home visits to 7-8 patients per day
• Deploy evidenced-based wound care interventions
• Determine appropriate follow-up frequency and manage ongoing wound care caseload
• Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners
• Engage with and appropriately utilize population health data to drive superior patient outcomes
• Attend ongoing educational opportunities
• Commute to assigned assisted living communities and patients’ private homes to provide care
