About The Position

Kindred Hospital Brea helps patients return to the lives they love. At our 48-bed hospital, our physician-led care teams provide critically ill patients with the specialized acute care and rehabilitation they need, for the time they need it - empowering them to take the next step in their recovery journey. We also offer a 4-bed ICU and three negative pressure rooms to accommodate particularly serious conditions. We are located less than a mile north of Brea Junior High School and conveniently surrounded by restaurants and shops. Implements policies and procedures, along with standards of care and practice related to patient care needs involving various types of wounds. Provides direct patient care and assistance to staff nurses and acts as consultative service regarding wound care to clinical staff.

Requirements

  • Graduate of an accredited nursing program leading to licensure as an LPN/LVN or RN
  • Current state RN or LPN/LVN license.
  • BLS certification.
  • Experience with wound care in any clinical setting.
  • Must read, write and speak fluent English.
  • Must have good and regular attendance.

Nice To Haves

  • ALS certification preferred
  • Wound Care Certificate preferred or obtain within 6 months of employment.

Responsibilities

  • Provides care for patients whose primary needs involve wounds.
  • Evaluates all wounds upon admission and ongoing to determine treatment plan and provide early problem identification.
  • Reviews initial treatment plan with the assigned nurse.
  • Reports to Nurse leadership any change in wound status.
  • Assists in planning patient's care.
  • Evaluates products and cost effectiveness for use in hospital and makes recommendation.
  • Reviews and participates in developing discharge plans.
  • Educates staff on products available for use in SAU.
  • Evaluates culture results and conferences with physicians on Plan of Care.
  • Serves as a consultant to facility staff and advocates with physicians; through training and support, enables clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care.
  • Makes recommendations to the physician for changes to wound care orders and provides evidence-based research support as needed.
  • Reviews charts of patients with wounds. Ensures that wound assessments, care plans, and treatments are clearly and correctly documented and that appropriate wound related treatments are being provided.
  • Assist with the investigation of cases with adverse events related to wounds.
  • Assist with the investigation of health care acquired pressure injuries.
