Wound Care Nurse Practitioner: Full-Time
About The Position
We are a dynamic mobile medical practice serving patients in private homes, nursing homes, and assisted living facilities throughout Livingston and surrounding areas. Our mission is to deliver compassionate, high-quality care directly to patients where they live, with a strong focus on wound management and geriatric wellness. We are seeking a Wound Care Nurse Practitioner (NP) to join our growing team. This role is ideal for a clinician who thrives in autonomous settings, values patient-centered care, and is passionate about improving outcomes for individuals with acute and chronic wounds.
Requirements
- Active Nurse Practitioner license in New Jersey
- Minimum 1 year of wound care experience (home health or facility-based a plus)
- Strong clinical judgment and ability to work independently
- Excellent communication and documentation skills
- Valid driver’s license and reliable transportation
Nice To Haves
- Certification in Wound Care (WOCN, WCC, or equivalent) preferred
Responsibilities
- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide advanced wound care, including debridement, dressing changes, and infection management
- Collaborate with physicians, caregivers, and facility staff to coordinate care
- Document visits and treatment plans accurately in EMR systems
- Educate patients and caregivers on wound prevention and care
- Travel to patient locations within Livingston and nearby communities
