Impact Health is looking for a Nurse Practitioner specializing in Wound Care to help cover our territory in Bangor & Augusta, ME. Requirements
- Active Nurse Practitioner (NP) license in the state of hire
- Proficiency in Electronic Health Records (EHR)
- Strong communication skills and a patient-centered mindset
Nice To Haves
- 2 years of experience preferred but not required
- Skilled Nursing Facilities (SNFs)
- Wound care and/or wound certification (WOCN, WCC, etc.)
- Long-term care, acute care, or rehabilitation settings
Responsibilities
- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide advanced wound care services, including debridement, dressing selection, and infection management
- Monitor healing progress and adjust treatment as needed
- Collaborate with facility staff to ensure continuity of care and prevention strategies
- Educate patients, families, and staff on wound management and prevention
- Complete documentation within 24 hours
- Ensure compliance with all state, facility, and clinical guidelines
