Wound Care Nurse Practitioner
About The Position
MDB Health Services is seeking a compassionate Wound Care Nurse Practitioner to provide specialized wound and foot care to residents in long-term care facilities. The role involves assessing wounds, creating individualized care plans, performing wound care procedures, monitoring healing progress, managing wound-related pain, and educating patients and facility staff. The practitioner travels to designated nursing homes in their area and communicates with facility staff, physicians, and other healthcare professionals to support coordinated care. The position offers part-time or full-time work and requires flexibility to see patients during weekday working hours.
Requirements
- Master’s or Doctoral degree in Nursing from an accredited program.
- Current Nurse Practitioner license in Louisiana.
- Minimum of 2 years of experience in wound care and/or foot care.
- Strong clinical skills in assessment and treatment of wounds and foot conditions.
- Independent, organized, and motivated approach to providing patient care.
- Excellent communication and interpersonal skills.
Nice To Haves
- Certification in Wound, Ostomy, and Continence (WOCN) is preferred but not required.
Responsibilities
- Provide wound care to residents in long-term care facilities.
- Conduct thorough wound assessments and develop personalized care plans for each patient.
- Perform wound debridement, dressing changes, and other wound care procedures as needed.
- Continuously monitor and assess patients’ progress in wound healing.
- Educate patients and facility staff on wound care techniques, prevention strategies, and best practices.
- Travel to designated nursing homes in the practitioner’s area.
- Communicate regularly with facility staff, physicians, and healthcare professionals to coordinate care and optimize patient outcomes.
- Maintain accurate and up-to-date patient records and documentation.
- Manage wound-related pain to ensure patient comfort.
- Maintain flexibility while seeing patients during weekday working hours.
