About The Position

The Wound Care Nurse-LPN has the responsibility and accountability for assessment, planning, implementation, and evaluation of wound care. Provides direct patient care and education to assigned patients. The Wound Care Nurse-LPN is actively involved in department activities, transdisciplinary team activities, and LifeCare activities to ensure individualized, patient-centered health care for all patient populations admitted to LifeCare Hospital. This role involves contact with patients in a clinical setting, requiring an understanding and demonstration of appropriate behavior when interacting with adult (18-65 years of age) and geriatric (65 and older) populations.

Requirements

  • Graduate of an accredited technical school of nursing is required.
  • Current Licensed Vocational/Practical Nurse license in the state where practicing is required.
  • Current BLS certification is required.
  • Maintain current licensure and BLS certifications while employed with LifeCare Hospitals.

Nice To Haves

  • Two (2) years of experience in an acute or LTACH setting is preferred.
  • ACLS certification within 6 months of hire is preferred.
  • CWON is preferred.

Responsibilities

  • Demonstrate knowledge and skills necessary to perform position requirements through successful completion of competencies established for the position, to include population served and other special needs of patients or customers, served by the department.
  • Adhere to LifeCare policies, procedures, all safety plans, and all standards imposed by regulatory organizations.
  • Under the supervision of the Wound Care Coordinator RN or MD, assess the needs of patients with basic and complex wounds.
  • Identify potential or actual needs and communicate changes in patient status, needs or condition to the assigned nurse and physician when applicable.
  • Document the assessment.
  • Initiate Skin Care Algorithm on all wound care patients and update as needed.
  • Develop comprehensive wound management plan and assist staff in wound care when indicated.
  • Document the plan and revisions.
  • Complete daily rounds and update the transdisciplinary team and family on wound progress.
  • Implement the plan of care by providing wound care interventions directly and/or appropriately delegating care to others, demonstrating competent and proficient nursing practice.
  • Respond appropriately and assist others in emergencies in a calm and effective manner.
  • Initiate and coordinate the evaluation of wound care provided and document the patients’ response to wound care by providing nursing and other members of the transdisciplinary team.
  • Obtain all pictures of wounds upon admission and weekly thereafter.
  • Identify individual learning needs of patients and families in wound care education in a manner that meets their special needs.
  • Communicate with the assigned nurse and physician on changing patient needs, ensuring patient safety at all times, notifying the assigned nurse of any risk factors seen, initiating, and maintaining appropriate safety measures.
