About The Position

The Wound Care Nurse works in harmony with the nursing staff to assess and treat patients with complex wounds, ostomies, and/or continence conditions and create care plans. The role provides consultation and direct assistance in managing patients with altered skin integrity, including pressure necrosis, wounds, and fistulas. The nurse also supports staff education and collects, analyzes, and documents wound-related information and statistics, and performs clinical evaluations for potential wound admissions.

Requirements

Must be a licensed LPN in good standing with the state, as required.
1–2 years of wound care experience required.
Reliable transportation required.

Nice To Haves

Wound Care Certification (WOCN) preferred.
1+ years of long-term care experience preferred.

Responsibilities

Provide consultation and direct assistance in managing patients with altered skin integrity, including pressure necrosis, wounds, and fistulas.
Offer in-service programs as needed to help nursing, rehabilitation, and medical staff maintain knowledge and competence in wound care.
Collect, analyze, and document information and statistics related to wounds in the facility.
Perform clinical evaluations for potential wound admissions.
