About The Position

The Inpatient Wound Care Program Manager, Advanced Certified is a master’s-prepared clinical leadership role responsible for providing wound care expertise, program development and oversight, and interdisciplinary coordination for the inpatient wound care program. This leader supports evidence-based prevention and treatment of acute, chronic, surgical, pressure-related, vascular, diabetic, and complex wounds across inpatient clinical areas. The role partners with Wound Medical Director, nursing, physicians, advanced practice providers, quality, infection prevention, nutrition, therapy, supply chain, and operational leaders to improve patient outcomes, reduce hospital-acquired pressure injuries, standardize wound care practices, develop and strengthen staff education and competency.

Requirements

  • Graduate of Accredited CWOCN/CWCN Nursing Education Program (wound scope, minimum) - Required
  • Master's Degree - in Nursing - Required
  • 3 Years - Direct patient care in acute care setting. - Required
  • Registered Nurse (RN) - State Licensure/Or Compact State Licensure - Required Upon Hire
  • Master's Level Degree in Nursing - Required Upon Hire

  • 1 Year - Experience as a Certified Wound Care Nurse. - Preferred
  • Certified Wound Care Nurse (CWCN) - Required within 1 year

Responsibilities

  • Provides direct patient care after analysis of patient status and needs to develop and evaluate individualized care plans by: applying WOC clinical knowledge and skills to analyze and assess patient needs; overseeing collaboratively developed individualized care plans for complex and specialized patients while providing guidance to staff; leading the evaluation of effectiveness of care plans through ongoing assessment, analysis, and revision, anticipating and resolving /mitigating issues and risks as they arise; and providing consultation to patients, families, and interdisciplinary team members to promote patient-centered care, fostering collaboration and communication.
  • Provides oversight and management of wound care staff: conducts recruitment, hiring and onboarding process and daily staffing to meet patient care needs; accountable for performance management of employees including annual performance reviews and follows progressive discipline process for time, attendance and performance deviations; identifies and intervenes with interpersonal or professional conflicts within and with the wound care staff; engages in discussions with wound care staff on their professional development needs including individual growth opportunities as well as mandatory required learning; monitors metrics and takes actions to enhance employee satisfaction; demonstrates ability to delegate tasks using clear communication and monitoring progress.
  • Maintain current knowledge of wound care research, professional standards, regulatory expectations, and emerging clinical best practices.
  • Demonstrates leadership skills by: identifying strengths and opportunities in self, team members and program; creating actionable items to address strengths/opportunities; and improve performance; adapting to competing demands, new responsibilities, change, challenges, and feedback; modeling team collaboration within and across teams; using effective communication skills; building relationships and mentoring staff.
  • Facilitates interdisciplinary collaboration by: leading collaborative efforts between nursing and various health care disciplines to drive the strategic implementation of evidence-based best practices, fostering a culture of consistency and continuity of care, and optimizing quality outcomes for patients; driving engagement with stakeholders to ensure seamless delivery of care, allocating resources effectively, and escalating issues as needed; and influencing the decision-making processes that affect patient care outcomes, fostering collaboration and communication among team members.
  • Review wound-related quality outcomes, identify trends, lead improvement initiatives, and partner with Quality and Patient Safety on data-driven action plans.
  • Oversees and coordinates the planning and delivery of skin and wound care education for patients, families, and staff by: leading the assessment and development of content to meet diverse learning needs, preferences, and cultural backgrounds; fostering partnerships with health care providers and external stakeholders to champion patient and staff education initiatives, cultivating high levels of engagement and participation; driving a culture of learning and development within the organization; allocating resources to support ongoing staff education and training in skin and wound care; and evaluating the effectiveness of these education programs.
  • Oversees business initiatives by: recognizing improvement opportunities and applying expertise in skin and wound care; collaborating to develop work plans to meet business priorities and deadlines; coordinating and delegating resources to implement work plan and accomplish organizational goals; monitoring progress and results; solving complex problems; escalating high-priority issues; recognizing and capitalizing on improvement opportunities; evaluating recommendations made.
  • Promotes compliance with regulatory standards and best practices by: engaging stakeholders to assess the impact of policy changes on patient care delivery and workflow processes, fostering collaboration and communication; providing expert input in policy/protocol creation, development, and review processes, ensuring alignment with regulatory standards, evidence-based practice, and organizational goals and objectives; and engaging with staff and stakeholders to evaluate the effectiveness of policies and procedures, resolving issues and making improvements as needed.
  • Serve as a resource for care transitions by supporting discharge recommendations, outpatient wound care referrals, home health coordination, and patient/family education.
