About The Position

HealWell, LLC is seeking a motivated and compassionate Advanced Practice Registered Nurse or Physician Assistant to provide specialized wound care across private residences, skilled nursing facilities, long-term care facilities, and outpatient clinic settings. Providers work independently while collaborating with patients, families, facility staff, and other healthcare professionals to create individualized treatment plans focused on wound healing, limb preservation, and quality of life. The role includes comprehensive patient and wound assessment, wound care procedures within the provider’s scope of practice, use of advanced wound products and therapies, vascular assessment and screening, coordination of referrals and follow-up, patient and caregiver education, and timely clinical documentation. The provider must follow company policies, HIPAA requirements, applicable regulations, and professional standards. The position also involves maintaining company-issued equipment and supplies and traveling to assigned patient locations. The ideal candidate is clinically confident, dependable, organized, and comfortable practicing with substantial autonomy. Experience with complex wounds and advanced wound care modalities is valued. Wound care certification is preferred; providers who are not certified may be required to obtain an approved certification within six months of hire.

Requirements

Graduate of an accredited Nurse Practitioner or Physician Assistant program.
Current, unrestricted APRN/PA license with prescriptive authority in Oklahoma.
Current national board certification.
Valid driver's license and ability to travel within the assigned service area.
Strong clinical assessment and decision-making skills.
Ability to work independently and manage a mobile patient schedule.
Excellent communication, organization, and documentation skills.
Comfortable providing care to medically complex patients in home, facility, and outpatient settings.

Nice To Haves

Two or more years of experience as an advanced practice provider.
Previous wound care experience; not required for the right candidate.
Wound care certification; providers who are not currently certified may be required to obtain an approved certification within six months of hire.
Experience with complex wounds, pressure injuries, diabetic ulcers, arterial and venous ulcers, surgical wounds, debridement, compression therapy, negative pressure wound therapy, and other advanced wound modalities.

Responsibilities

Perform comprehensive patient and wound assessments.
Develop and manage individualized wound care treatment plans.
Assess, measure, stage, and document wounds and wound characteristics.
Perform wound care procedures within scope of practice, including selective and excisional debridement as appropriate.
Use advanced wound care products, therapies, and treatment modalities.
Assess vascular status and perform or interpret vascular screening, including ABI/TBI when indicated.
Identify barriers to wound healing and coordinate appropriate referrals, diagnostic testing, and follow-up care.
Collaborate with physicians, nursing staff, home health agencies, facility staff, patients, families, and other healthcare team members.
Educate patients, caregivers, and facility staff about wound care, pressure injury prevention, nutrition, offloading, compression, and factors affecting wound healing.
Maintain accurate, thorough, and timely clinical documentation.
Practice in accordance with HealWell policies, HIPAA requirements, applicable state and federal regulations, and professional standards of practice.
Maintain company-issued equipment and wound care supplies.
Travel to assigned patient locations as required.
