About The Position

The Case Manager plays a pivotal role in coordinating and delivering comprehensive support services to individuals and families in need, ensuring their well-being and stability. This position involves assessing client needs, developing personalized care plans, and facilitating access to appropriate resources and services within the community. The Case Manager acts as a liaison between clients, healthcare providers, social service agencies, and other stakeholders to promote effective communication and continuity of care. A critical outcome of this role is to empower clients through advocacy, crisis intervention, and ongoing support, ultimately improving their quality of life and fostering long-term self-sufficiency. The Case Manager also contributes to program development and evaluation to enhance service delivery and address emerging community needs.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or 6 years equivalent experience.
  • Minimum of two years of experience in case management or social services.
  • Knowledge of crisis intervention techniques and mental health support strategies.
  • Familiarity with child welfare systems and discharge planning processes.

Responsibilities

  • Conduct comprehensive assessments of clients’ social, emotional, and health needs to develop individualized care and discharge plans.
  • Coordinate and monitor access to social services, mental health resources, and community programs to support client goals.
  • Provide crisis intervention and conflict resolution to address immediate client challenges and ensure safety.
  • Collaborate with multidisciplinary teams including healthcare providers, social workers, and child welfare agencies to ensure integrated service delivery.
  • Document client progress, maintain accurate case records, and prepare reports to support program evaluation and compliance.
  • Advocate on behalf of clients to secure necessary resources and navigate complex service systems.
  • Participate in program development initiatives to improve service effectiveness and respond to community trends.
