About The Position

Under the direction of the Director of Clinical Services, the Women’s Health RN Case Manager is responsible for providing case management services for Women’s health services, including but not limited to services related to prenatal health and the Breast and Cervical Cancer Program (BCCP). The responsibilities of the RN Case Manager include providing counseling, education, and service coordination to patients eligible for the program and assisting in all data collection and reporting. The RN Case Manager serves as an advocate for women’s health services and works with other healthcare systems to enhance continuity of care and treatment. This is a grant-funded position and continued employment is dependent on the availability of grant funds.

Requirements

  • Bachelor's or associate degree in Nursing
  • Must have knowledge of social service and health care delivery systems
  • Must have at least one year of nursing experience in a community health, hospital, or health education related setting
  • Valid license as a Registered Nurse in Mississippi required
  • Current Basic Life Support (BLS) through American Heart Association
  • Must be able to continuously use sound communication and counseling skills in handling title case management activities
  • Must be able to reason in planning and reporting
  • Must be able to communicate effectively both orally and in writing
  • Must be able to deal effectively with patients and staff

Responsibilities

  • Performs and documents case management assessments and required screenings (family planning needs, mental health and substance use screenings, nutritional screening, surveys for social disparities and needs, care plan, and self-management skills.
  • Generates and documents face-to-face, telephone, and other patient-related encounters in the patient’s electronic health record within 24 hours of the date of service.
  • Coordinates outreach activities to distribute BCCP and other women’s health related education and materials to educate the community about available services.
  • Attends meetings and trainings for the BCCP and participates on the BCCP, Title X and other related committees.
  • Effectively oversees an assigned caseload of patients including the scheduling of clinic and specialty care visits, labs, screenings, vaccines, medication needs, socioeconomic needs, and maintains current case management paperwork
  • Maintains accurate data files regarding assigned patients including EHR documentation and paperwork required by Mississippi Department of Health for the BCC program.
  • Completes data entry and submission for all grant funded services, labs and screening labs results, screenings, immunizations, referrals, and other required data to all required agencies.
  • Completes internal and external referrals for patients when medically indicated, follows up on the outcome of the referrals and documents it in EHR.
  • Conducts patient navigation services after receipt of referral and screening or diagnostic results and documents services provided in the EHR.
  • Participates actively in Performance Improvement activities and monitors problems discovered
  • Works closely with the clinic providers to investigate contributing factors in a patient’s illness and needs for continuity of care and optimal health outcomes.
  • Completes and submits all monthly reports for services provided and program enrollment to the Director of Clinical Services by the 10th of each month.
