About The Position

The Home Health RN Case Manager develops and maintains a therapeutic relationship throughout the healthcare continuum. This role involves providing individualized care to promote physical and psychosocial comfort, assessing patient and family readiness, and involving them in planning and implementing care. The position requires mobilizing appropriate resources, adapting practice to evidence-based literature, and participating in quality improvement initiatives. The RN Case Manager will also educate patients and families, assist in creating an environment for educational growth for peers, and utilize the nursing process as a problem-solving model. This includes recognizing clinical changes, evaluating and implementing changes to the plan of care, making sound clinical decisions for complex patients, and prioritizing patient care. Initiating referrals and discharge planning, applying ethical decision-making, monitoring confidentiality, and delegating correctly to unlicensed assistive personnel are also key responsibilities. The role demands collaboration with peers and other disciplines, demonstrating leadership behaviors, and committing to the development of self and others. Participation in unit meetings, utilizing resources for knowledge, and demonstrating effective communication, feedback, and conflict resolution skills are essential. The position requires critiquing one's own nursing practice and achieving required certifications or a Bachelor's Degree, along with earning a minimum of 10 contact hours per year.

Requirements

  • Graduate from an accredited school of nursing
  • Earn 10 continuing education credits annually
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
  • Ability to work weekends, holidays, and different shifts in order to accommodate staffing
  • Ability to take “on call” assignments.
  • Ability to handle multiple tasks and complete work within short timeframes.
  • Ability to transport objects, patients, and equipment
  • Ability to respond quickly to patient needs and work at fast pace
  • Ability to respond to patients in a timely manner and access all areas of the unit
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
  • Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
  • Must be willing to work occasional overtime if needed.
  • Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
