Bi-Bett Corporation is a California nonprofit with more than 50 years of experience providing treatment and recovery services to adults with substance use and co-occurring disorders. Originally based in Contra Costa County, Bi-Bett now operates residential and outpatient programs throughout Alameda and Solano Counties. Position Summary The Women’s and Children’s Case Manager provides comprehensive, on-site case management services to women and their children residing in a residential substance use disorder (SUD) treatment program. The Case Manager is an integral member of the multidisciplinary treatment team and assists clients in addressing practical, social, family, healthcare, housing, childcare, transportation, employment, education, and financial needs that may affect successful treatment and long-term recovery. The Case Manager advocates for clients and their children while promoting self-sufficiency, family stability, recovery, and successful transition to community-based services. The position requires a proactive approach to identifying barriers, coordinating resources, following up on referrals, and helping clients develop the skills necessary to independently navigate community systems. Services are provided using a trauma-informed, person-centered, strength-based, culturally responsive, and recovery-oriented approach.

Requirements

  • High school diploma or equivalent required
  • Experience in case management, social services, behavioral health, SUD treatment, residential treatment, or a related field.
  • Knowledge of community resources and social-service systems in Contra Costa and Alameda Counties.
  • Experience or demonstrated ability to work effectively with women, families, and individuals experiencing substance use, housing, healthcare, financial, or other social barriers.
  • Strong written, verbal, organizational, interpersonal, and problem-solving skills.
  • Ability to prioritize multiple client needs, follow through on referrals, and maintain accurate documentation.
  • Ability to maintain confidentiality, professional boundaries, and appropriate client relationships.
  • Ability to work effectively as part of a multidisciplinary treatment team.
  • Ability to advocate for clients while promoting independence and self-sufficiency.
  • Ability to work primarily on-site in a residential treatment environment and interact professionally with clients, children, staff, visitors, and community providers.

Nice To Haves

  • Associate or Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Behavioral Health, or a related field preferred.
  • Experience working with women and children, pregnant women, or mothers in a treatment or social-service setting preferred.
  • Experience working with county agencies, child welfare systems, healthcare providers, or community-based social services preferred.
  • Knowledge of Medi-Cal, CalFresh, WIC, SSI/SSDI, housing programs, childcare resources, and other public benefits preferred.
  • Experience with discharge planning, community referrals, and coordinating services preferred.
  • Knowledge of trauma-informed care, recovery-oriented services, and co-occurring mental health and substance use disorders preferred.

Responsibilities

  • Provide consistent, accessible, and ongoing case management services to residents throughout their treatment episode.
  • Conduct case management needs assessments and identify client and family needs following admission.
  • Meet individually with clients to establish, monitor, and update individualized case management goals.
  • Assist clients with applications, forms, identification documents, benefits, referrals, appointments, and other documentation necessary to access services.
  • Help clients navigate healthcare, housing, childcare, transportation, employment, education, public benefits, legal services, and other community resources.
  • Develop and maintain knowledge of local resources and services available to women, children, and families.
  • Coordinate appointments and referrals with healthcare providers, social-service agencies, county programs, schools, childcare providers, and other community organizations.
  • Follow up with clients and service providers to ensure identified needs and referrals are addressed.
  • Participate in multidisciplinary treatment team meetings, case conferences, discharge planning meetings, and other program meetings as assigned.
  • Communicate relevant case management information to the treatment team while maintaining client confidentiality and appropriate professional boundaries.
  • Identify and address social, financial, family, housing, transportation, healthcare, and other barriers that may interfere with treatment participation or successful discharge.
  • Advocate for clients while encouraging participation, decision-making, self-advocacy, and personal responsibility.
  • Coordinate services for children, including healthcare, dental care, education, childcare, developmental services, behavioral health, and other identified needs.
  • Assist mothers with parenting, family-related, childcare, and family-support resources.
  • Provide referrals to parenting education, family support, domestic violence, legal, and other appropriate services when indicated.
  • Support family stability and reunification efforts when appropriate and consistent with the client’s treatment and discharge plan.
  • Collaborate with Counselors, Clinicians, Medical Staff, Program Coordinators, and other members of the treatment team.
  • Assist the treatment team in developing realistic and individualized discharge and aftercare plans.
  • Begin discharge planning early in the treatment episode and coordinate appropriate services prior to discharge whenever possible.
  • Assist clients with connecting to continuing SUD treatment, mental health services, healthcare, housing, recovery residences, transportation, employment, education, childcare, public benefits, and community-based recovery supports.
  • Complete accurate and timely documentation of assessments, contacts, referrals, case management activities, treatment-team participation, and discharge planning.
  • Maintain strict client confidentiality and comply with applicable privacy requirements, including HIPAA and 42 CFR Part 2, as applicable.
  • Maintain a professional, safe, supportive, and visible presence within the residential treatment environment.
