About The Position

Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc. “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care. Under the direction of the WIC Director, the WIC Community Coordinator develops and maintains community liaison with health and human services providers and advocacy organizations and communities served by the local WIC program. The WIC Community Coordinator builds an active, coordinated referral network and collaborative, working relationships between community agencies, groups and WIC to promote the utilization of WIC services. The WIC Community Coordinator assists with retention activities to strengthen the communication with WIC participants about their eligibility with particular focus on current enrollment status and recertification.

Requirements

  • High School or associate’s degree graduate.
  • At minimum a High School diploma.
  • Knowledge of community and neighborhoods served by the local WIC program.
  • Fluency in English.
  • Two years of experience in community work, outreach, or health and human services preferred.
  • High level of customer service and a positive approach to work with patients.
  • Projects positive attitude about the department and the health center, and offers services to ensure positive experience.
  • Excellent oral and written communication skills.
  • Time management skills.

Nice To Haves

  • Bilingual in Portuguese/CV Creole or Spanish preferred (specific bilingual capacity needed as appropriate to the community served by the local WIC program).

Responsibilities
