About The Position

The Wellness Team Supervisor is responsible for the day-to-day supervision of care staff in assisted living and memory care, ensuring caring and personalized assistance for residents. This role involves leading, managing, and being accountable for the team's performance and upholding the company's 1440 care standards. The position also focuses on employee experience and fostering a culture of leadership and service.

Requirements

  • High School Diploma or GED preferred.
  • Previous Healthcare, Geriatrics or experience working with the elderly population preferred.
  • 2 years Leadership Experience
  • Certified Nursing Assistant, Licensed Practical Nurse, or Registered Nurse with at least 2 years of related experience or training; or equivalent combination of education and experience.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Occasionally lift and/or move up to 25 pounds.

Responsibilities

  • Lead, manage, and be accountable for your team and their performance.
  • Ensure timely and accountable attendance, upholding work schedules and being ready to start work on time.
  • Understand and lead the 1440 care standards for residents and families.
  • Provide direct supervision of at least ten or more full-time community care staff, including monitoring job performance, conducting evaluations, coaching, counseling, and documenting corrective action.
  • Participate in hiring, firing, and disciplinary processes and decisions.
  • Make daily rounds to ensure resident care staff perform assignments according to task sheets and acceptable care standards, laws, and regulations.
  • Review resident care notes for completeness, ensuring they are descriptive and reflect resident responses.
  • Verify all associate hours in NovaTime and weekly hour reports.
  • Complete resident care associate schedules to ensure proper coverage according to community needs and budget.
  • Coordinate monthly staff schedules, arrange replacement staffing, and maintain clear records of changes.
  • Develop and maintain resident caregiver task sheets daily.
  • Perform daily and monthly safety and infection control checks.
  • Maintain appropriate and complete documentation per company policies.
  • Train resident care staff or coordinate training programs.
  • Complete new hire orientation with all staff.
  • Monitor completion of monthly Relias training for all care staff and assign additional trainings as needed.
  • Respond promptly to all emergency calls from communities.
  • Share on-call duties as required.
  • Be available to work weekends and/or be on call as assigned.
  • Work toward continual improvement of the overall organization.
  • Participate in community functions including family socials and open houses.
  • Perform other duties as assigned.
