About The Position

Are you a detail‑oriented nurse who believes great healthcare is built on relationships, advocacy, and teamwork? As an ID/DD Wellness Nurse, you’ll play a vital role in supporting the health, safety, and quality of life of individuals with Intellectual and Developmental Disabilities. In this role, you’ll coordinate care, conduct wellness assessments, track health outcomes, and collaborate with families, providers, and support staff to ensure every individual receives thoughtful, proactive, and person‑centered care. You’ll need strong clinical judgment, excellent documentation skills, and a heart for service—along with the ability to work independently while being an active member of a multidisciplinary team.

Requirements

  • Valid Indiana RN or LPN License
  • CPR certification
  • Free from communicable diseases
  • Valid Indiana driver’s license
  • Reliable transportation with current Indiana automobile insurance
  • Clean criminal background, OIG record, and aide registry
  • Successful completion of required training and orientation
  • Strong nursing, assessment, and observation skills
  • Excellent written and verbal communication
  • Ability to document accurately and efficiently
  • Strong organization and problem‑solving abilities
  • Ability to work independently with minimal supervision
  • Knowledge of Medicaid and Medicaid Waiver regulations (especially Waiver Services)
  • Ability to interact effectively with individuals of varying abilities, families, guardians, and professionals
  • Basic proficiency with Word and relevant software

Responsibilities

  • Coordinate and oversee comprehensive wellness services for individuals served
  • Complete annual wellness assessments and individualized wellness plans
  • Collaborate with physicians, case managers, behavior consultants, guardians, and Direct Support Professionals
  • Track and document key wellness indicators, including: Height, weight, and BMI, Annual flu vaccinations, Annual physician exams, Annual dental visits
  • Actively work toward measurable improvements, including: 5% reduction in medication administration errors over 12 months, 5% reduction in choking incidents requiring intervention, 5% reduction in fall-related incidents requiring intervention
  • Provide ongoing RN/LPN consultation and face‑to‑face visits based on individual health needs: Tier I: Weekly review with at least one face‑to‑face visit monthly, Tier II: Weekly review with two face‑to‑face visits monthly, Tier III: Weekly review with weekly face‑to‑face visits
  • Maintain accurate, timely, and thorough documentation in accordance with Medicaid and Waiver regulations
  • Ensure compliance with service definitions and re‑approval requirements
  • Use assessment data to drive individual health goals and outcomes
