Requirements

A positive attitude and a team-first mindset.
Strong communication skills —especially over the phone and via text.
Comfort with technology, including Microsoft Office and electronic medical records.
Ability to multi-task, prioritize, and stay organized in a fast-paced environment.
High school diploma or equivalent required.

Nice To Haves

A basic understanding of medical terminology is a plus.
Associate Degree preferred.
Prior clerical experience in a healthcare setting is preferred.

Responsibilities

Make outreach calls to patients, guiding them through eligible preventive care services like Medicare Annual Wellness Visits.
Educate patients on the importance of preventive care and help close care gaps that impact long-term health.
Work side-by-side with physicians, nurses, and staff to create a seamless, integrated approach to patient care.
Support primary care practices in achieving better outcomes through documentation, process enhancements, and occasional presentations at staff meetings.
Assist with general office tasks, secretarial duties, and population health initiatives that make a real difference in our communities.
