About The Position

Bridge Senior Living is hiring a Wellness Nurse to implement and coordinate resident care in collaboration with physicians and care associates. The nurse will help assess, plan, implement, and evaluate resident care, and contribute to a positive environment for residents. The schedule is Fridays and Saturdays from 7 a.m. to 7 p.m.

Requirements

• High school diploma or equivalent (GED).
• LPN license in the applicable state.
• First Aid and CPR certification.
• At least one year of related work experience.

Responsibilities

• Assess, plan, implement, and evaluate residents’ care.
• Make frequent rounds to provide a high level of care and ensure continuity of care.
• Document pertinent care information and observe residents’ overall condition and behavior.
• Assist the Director of Wellness with developing and updating resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding resident care.
