About The Position

The Wellness Nurse provides care to residents in an assisted living community, supporting their health and well-being through assessments, medication administration, care planning, monitoring, and accurate documentation. The role involves collaborating with residents, families, and healthcare professionals, advocating for residents’ rights and preferences, and responding to medical emergencies. This is a per-diem position across all shifts, with weekend availability required. A criminal background screening is required.

Requirements

Hold a valid, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
Communicate effectively with residents, families, and healthcare professionals, including explaining medical information clearly.
Analyze situations, make sound decisions, and provide appropriate solutions in a timely manner.
Demonstrate high ethical standards, respect residents’ privacy, and maintain professionalism.
Have strong record-keeping skills.
Provide care with kindness and empathy.
Be available to work weekends.

Responsibilities

Collaborate with a team to develop, implement, and review personalized resident care plans.
Conduct health assessments and identify medical concerns or changes in residents’ conditions.
Work with other healthcare professionals to develop effective care plans.
Administer prescribed medications accurately and in accordance with protocols.
Monitor medication effectiveness and address side effects promptly.
Maintain accurate medical, care-plan, and incident records to support regulatory compliance and consistent care.
Respond to medical emergencies and coordinate with emergency services when needed.
Communicate with residents’ families about residents’ health and changes in condition.
Advocate for residents’ rights and preferences.
