About The Position

The Wellness Nurse provides care to residents in an assisted living community, addressing each resident’s needs while maintaining accurate records and conducting regular checks. The role supports residents’ well-being and independence and contributes to the community’s standard of care. A criminal background screening is required.

Requirements

• Valid, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• Ability to communicate medical information clearly with residents, families, and healthcare professionals.
• Ability to analyze situations, make sound decisions, and provide appropriate solutions promptly.
• High ethical standards, respect for residents’ privacy, and professionalism.
• Strong record-keeping skills.
• Kindness and empathy in providing care.
• Weekend availability.
• Criminal background screening required.

Responsibilities

• Collaborate with a team to develop, implement, and review personalized resident care plans.
• Conduct health assessments, identify medical concerns or changes, and work with healthcare professionals to create effective care plans.
• Administer prescribed medications accurately and according to protocols; monitor their effectiveness and respond to side effects.
• Maintain accurate medical, care-plan, and incident records for regulatory compliance and continuity of care.
• Respond to medical emergencies, provide immediate care, and coordinate with emergency services when needed.
• Communicate with residents’ families about residents’ health and changes in condition.
• Advocate for residents’ rights and preferences.
