About The Position
Bridge Senior Living is hiring a Wellness Nurse to implement and coordinate the delivery of care in collaboration with physicians and care associates. The role focuses on serving seniors and contributing to a positive environment for residents. The schedule requires weekend and holiday availability.
Requirements
- High school diploma or equivalent (GED).
- LPN license in the applicable state.
- First Aid and CPR certification.
- At least 1 year of related work experience.
- Weekend and holiday availability.
Responsibilities
- Assess, plan, implement, and evaluate resident care.
- Make frequent rounds and provide a high level of care while ensuring continuity of care.
- Document pertinent care information and observe residents’ overall condition and behavior.
- Assist the Director of Wellness with developing and updating resident care plans.
- Maintain ongoing communication with residents, families, and physicians regarding residents’ care.
