About The Position

Enrich the lives of older adults through community, choice, and continuing care in the LPN Wellness Nurse role. The Wellness Nurse promotes safety and wellbeing, supports transitions in care level, and provides medically-related services to independent residents throughout the retirement community. This is accomplished through nursing assessments, early intervention, and ongoing health education. Additionally, the Wellness Nurse plays a key role in educating residents about the full scope of care services available throughout Wesley. Find your sense of belonging at Wesley!

Requirements

  • Associate’s degree in Nursing or related field.
  • Active Licensed Practical Nurse (LPN) license in Washington State or MSL compact.
  • Ability to obtain a CPR and First Aid certification within 30 days of hire.
  • Effective and comprehendible English verbal and written communication skills to accomplish charting, record keeping, and to comprehend written orders and procedures.
  • Displays a personable and comforting personality to care for the residents.
  • Use logic and reasoning to identify alternative solutions, conclusions, or approaches to problems.
  • Ability to give direct patient care, including participating in lifting, transferring, positioning, and assistance of residents with walking or in wheelchairs.
  • Able to lift 50lbs as needed.
  • Ability to use a computer keyboard or touch screen to document resident records.
  • Apply your physical and muscle strength to lift, push, pull or carry objects.
  • Ability to utilize your lower back and abdominal muscles to support part of the body repeatedly and continuously over time without fatiguing.
  • Ability to listen to and comprehend information and ideas provided through conversation, email, and text.

Responsibilities

  • Meet with new residents and present the Notice of Services disclosure.
  • Serves as the primary point of contact for nursing questions and services for independent and catered living residents within a retirement community.
  • Provide wellness services to residents such as blood pressure checks, health education, communicable disease tracking, medication management, or administration.
  • Provides and support diabetic management which may include coordinating with nurse delegator at times.
  • Review clinical notes for residents moving to or returning from a higher level of care, and convey potential care needs to the Home Care Manager.
  • Understand in Home Care Services regulations and strives to meet and abide by these policies and procedures.
  • Track and supports residents' transition back to the retirement community following an offsite stay or visit to a hospital or skilled nursing facility.
  • Coordinates with hospital, skilled nursing facility, discharge planner or social worker to ensure appropriate care upon return home.
  • Collaborates with campus staff, Executive Directors, and Home Care Managers in ensuring appropriate levels of care are in place for Residents.
  • Coordinates care conferences with residents, Power of Attorneys, and relevant campus staff when potential transition to higher level of care is necessary.
  • Responds to pendant calls on campus if no home care aide is available.
  • Responds to emergency calls.
  • Coordinates On-Campus clinics with outside providers such as; foot & nail care, hearing aide services, dental services, dermatology, wound care, etc.
  • Participates in Resident Review meetings.
  • Conducts resident safety checks as needed.
  • Completes timely and accurate incident reports in electronic medical records system.
  • Provides at least one follow up resident visit for post-accident and injury, 911 call, or hospitalization.
  • Maintain accurate and up-to-date health records in electronic medical records system.
  • Other duties and special projects as assigned.
