About The Position

Under the direction of the Assisted Living Manager, provide highly responsible wellness services in accordance with established assisted living policies and procedures. This role involves completing assessments, developing and updating service plans, providing first aid, coordinating with third-party services, documenting resident care, managing supplies and medications, and supporting unlicensed staff. The position operates within the scope of practice for an LPN in Arizona and involves direct resident care, including potential lifting and moving of residents.

Requirements

  • Thorough knowledge of professional nursing practices/theories, techniques and tactics.

  • Computer proficiency-- Microsoft Word/Excel, Outlook and Internet.
  • Organized, able to multi-task, think creatively and adapt to different situations.
  • Ability to react successfully and calmly to any situation that may present itself with a resident, guest, or staff.
  • Has ability to write routine reports and correspondence.
  • Must be able to complete training on computer and/or other devices.
  • Maintain competency in the position and knowledge of trade/industry advances and new technologies.
  • Must be able to effectively communicate in English with all levels of staff, residents, and guests.
  • Current Arizona State Board of Nursing Licensed Practical or Registered Nurse Licensure.
  • Has current and valid Fingerprint clearance card or apply for one within 20 days of hire.
  • Has current Basic Life Support (BLS) certification obtained in a classroom setting and in compliance with or endorsed by American Heart Association, American Red Cross, or National Safety Council guidelines.
  • Has a Certificate of Completion for memory care services training or obtains within 30 days of hire and complies with annual certification requirements.
  • Is able to carry/lift/push/pull up to 30 pounds occasionally.
  • May need to operate a fire extinguisher.
  • Is able to assist in evacuation procedures.
  • Can tolerate exposure to physical hazards including potential exposure to bodily fluids and associated pathogens/infectious disease, as well as potential exposure to chemicals.
  • Physical: kneeling, lifting/moving, carrying/transporting, walking/traversing or moving, sitting/stationary position, standing/stationary position, bending, reaching to include forward and above shoulder level, pulling, pushing, fine manipulation (pen, using a mouse), gross/total manipulation (firm hand grasping, twisting), twisting, stooping.
  • Cognitive: direct others, written and oral communication, work under time constraints, concentrate amid distractions, math/calculations, ensure accuracy of details, creativity, remember names and details, problem solve, make decisions, work rapidly, examine/observe details, analytical ability, working within minimal errors, reason (make sense of things).
  • Environmental: inside, odors, fumes, near moving mechanical parts, working with others, working around others, working alone.

Nice To Haves

  • Preferably a two year degree from an accredited college or university in nursing, public health, or related field.
  • Preferably three to five years of progressive responsibility in a health care environment; or a combination of education, training and experience that would ensure the effective execution of the duties of the job.

Responsibilities

  • Completes assessments ensuring appropriate level of care is determined and provided.
  • Completes Service Plans within the appropriate timeframe per the regulations including auditing during service plan intervals as needed and/or when there is a change of condition with the resident.
  • Performs appropriate initial or ongoing screenings to determine appropriate levels of care and other physical or cognitive needs of potential or current residents.
  • Updates Service Plans per the appropriate required timeframes for Personal and Directed levels of care that include amount, type and frequency of services.
  • Meets with resident, responsible parties and the AL Manager for scheduled Service Plan meetings as needed.
  • Provides first aid to residents when appropriate and requests physician orders for on-going treatment or intermittent nursing management of a problem that can be safely managed in the assisted living environment.
  • Actively participates in identifying the need for, and acquiring, third party services such as Hospice, Home Health, PT, OT, ST, or SN when a resident has a condition that requires such services.
  • Completes documentation in the resident record when responding to any emergent situations and incidents.
  • Maintains treatment room stock items in an orderly manner, and charges items out per facility protocol using the Inventory Management electronic system.
  • Responsible for entering or verifying orders in a timely manner following policy and procedures.
  • Administer medication, treatments and biological to residents under physician’s orders and in compliance with accepted pharmacy and the assisted living policies and procedures.
  • Follow facility policies and procedures for reporting and documenting any incidents or variances of standard operations.
  • Ensures that medications and supplies are maintained with adequate levels according to established orders and procedures.
  • Ensures that medication cabinets are routinely monitored for discontinued or expired medications and handle disposition of such in an appropriate manner.
  • Controlled medications or treatments are maintained and accounted for daily in locked storage area in appropriate medication rooms as needed.
  • Ensures that caregivers are confirming signing off on completed tasks in EMR daily. Progress notes are completed to support the ongoing care, services and interventions provided to residents.
  • Provides support and assistance, advice and guidance to unlicensed staff in the assisted living setting, reporting any concerns or needs to the appropriate manager.
  • Correctly query, modify, and create all information in the resident’s electronic record gathered during the shift.
  • Ensures that the resident’s medical or health problems, including physical, behavioral, cognitive, or functional conditions or impairments are supported as addressed in the Service Plan as required per R9-10-808.
  • When necessary, lifts, moves, and transports residents, using proper body mechanics or lifting devices.
  • Perform other duties as assigned.
  • Supervises non-exempt staff.
