About The Position

The Wellness Coordination Manager enthusiastically embraces and promotes the mission and values of the organization through all actions while overseeing the delivery and integrity of Wellness Coordination services. This role proactively coordinates healthcare services for clients to support optimal health outcomes and quality of life. The Wellness Coordination Manager oversees the development and maintenance of client health plans, collaborates with interdisciplinary teams, provides clinical guidance and training to staff, and ensures health-related support is delivered in accordance with organizational standards, regulatory requirements, and person-centered practices. The position also provides oversight and support to nursing staff and serves as a resource to clients, teams, and stakeholders regarding healthcare coordination and wellness services.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred; Associate of Science in Nursing (ASN) with relevant nursing experience considered. Equivalent combinations of education and experience may be considered.
  • Must possess and maintain an active Indiana Registered Nurse (RN) license in good standing.
  • Current CPR certification is required.
  • Core A and Core B Train-the-Trainer certifications required or ability to obtain within designated timeframe.
  • Minimum of one to two years of nursing or healthcare coordination experience preferred, including experience related to the responsibilities of the position.
  • Experience supporting individuals with intellectual and developmental disabilities or within community-based services preferred.
  • Demonstrated ability to coordinate healthcare services, maintain clinical documentation, provide staff training, and collaborate effectively with interdisciplinary teams.
  • Strong organizational, communication, problem-solving, and leadership skills are required.
  • Ability to maintain professionalism, confidentiality, and effective working relationships with clients, families, staff, healthcare providers, and community partners.
  • Must possess a valid driver’s license, maintain an acceptable driving record, meet agency driver insurability requirements, and maintain reliable transportation with current automobile insurance coverage.
  • Must successfully complete and maintain all required agency training, regulatory training, and continuing education requirements necessary to support compliance with applicable regulations, standards, and best practices.

Responsibilities

  • Promotes positive health outcomes for clients through oversight and continuous improvement of Wellness Coordination and clinical support services.
  • Maintains an assigned Wellness Coordination caseload and provides direct Wellness Coordination services to clients as needed, including assessments, monitoring, care coordination, documentation, and participation in interdisciplinary planning.
  • Provides leadership, guidance, and oversight to assigned clinical staff, including: Review and oversight of LPN documentation and clinical notes. Approval of staff time off requests, expenses, and performance evaluations
  • Collaborates with organizational leadership to develop, implement, and maintain policies, procedures, and best practices related to Wellness Coordination and health services.
  • Participates in administrative, leadership, and committee meetings as required.
  • Ensures clinical and Wellness Coordination services are delivered in compliance with all applicable federal, state, waiver, and organizational regulations and standards, including: Indiana Administrative Code (IAC 460), FSW/CIH Waiver requirements, Wellness Coordination service standards and regulations, CARF International accreditation standards
  • Participates in interdisciplinary team meetings, case conferences, and client reviews through direct involvement and/or written reports.
  • Complete and oversees required clinical and service documentation, including but not limited to concurrent reviews, quarterly reviews, annual reviews, medical reviews, plan of care documentation, and nursing summaries.
  • Assists in the development, implementation, and evaluation of person-centered plans related to health, hygiene, nutrition, wellness, and risk reduction.
  • Performs client admission assessments.
  • Provides training, education, and consultation to staff and clients on health-related topics, medication administration, wellness support and identified clinical needs.
  • Develop and oversee high-risk health plans as needed and ensure appropriate staff training and follow-through related to client medical and wellness needs.
  • Coordinates and monitors medical, dental, behavioral, pharmacy, and other healthcare services to ensure client health needs are effectively addressed.
  • Maintains effective communication and collaboration with clients, families, guardians, physicians, pharmacies, interdisciplinary teams, service providers, and community partners.
  • Monitor the medication administration of all clients. Assist in education of clients regarding self-administration of medications.
  • Ensures compliance with all agency policies, procedures, and documentation standards.
  • Performs other duties and responsibilities as assigned in support of organizational operations and client services.
