The Wellness Coach will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow up. The role involves providing case management, developing and monitoring individual support plans, assisting with problem-solving and crisis intervention, and acting as an advocate for consumers. The coach will also identify and assist with daily living skills, support facilities management, provide supportive counseling, conduct home and hospital visits, and maintain strong working relationships with therapeutic support services. Documentation of services, assisting with moves (including lifting, packing, and cleaning), and participating in team activities are also key responsibilities. The position requires liaison with community-based services, participation in supervision and meetings, and adherence to an on-call system for crisis intervention.
