About The Position

At NIVA Health, we’re looking for a Wellness Case Manager to help guide patients through their wellness journey — from their first interaction with our clinic through ongoing programme engagement. This role is ideal for someone who thrives in a people-focused, fast-paced environment and enjoys combining relationship-building, patient support, organisation, and measurable performance goals. You’ll work closely with providers and clinic leadership to help patients stay engaged, informed, and connected to the services designed to improve their overall wellness experience.

Requirements

  • High School Diploma or equivalent required
  • Must be available to work full-time on-site within an assigned NIVA Health clinic
  • Strong communication skills

Responsibilities

  • Building strong, trust-based relationships with patients
  • Supporting patient engagement and wellness programme participation
  • Guiding patients through provider-directed services and next steps
  • Coordinating follow-ups, scheduling, and patient communications
  • Helping create a seamless, elevated patient experience
  • Tracking personal KPIs related to engagement, follow-up activity, and conversion support
  • Supporting new wellness initiatives and service rollouts within the clinic
  • Working closely with providers and clinic staff in a collaborative environment
