About The Position

Concierge Home Care is seeking an experienced Licensed Practical Nurse (LPN) with strong home health and triage experience to join their growing Virtual Care team. This role is for a 100% work-from-home position, requiring candidates to reside within the state of Florida. The nurse will play a key role in reducing hospitalizations and improving patient outcomes through proactive remote support and clinical intervention. The position offers a consistent weekend schedule.

Requirements

• Candidates MUST reside within the state of Florida due to operational, licensing, and/or business requirements.
• Active, unencumbered Florida LPN license
• Minimum of 2 years of home health experience
• Experience with triage and hospitalization prevention
• Strong clinical judgment and communication skills
• Comfortable using telehealth platforms and EMR systems
• Reliable internet connection and private remote workspace

Nice To Haves

• Experience working with high-risk or medically complex patients
• Knowledge of chronic disease management
• Microsoft Office proficiency

Responsibilities

• Conduct virtual patient visits and clinical assessments
• Perform triage and identify early warning signs or risk trends
• Support chronic disease management and patient education
• Monitor telehealth data and escalate concerns appropriately
• Help prevent avoidable ER visits and rehospitalizations
• Collaborate with interdisciplinary care teams
• Document patient interactions accurately and timely
• Participate in care planning and performance improvement initiatives
• Support development of virtual care best practices
