About The Position

Primary functions are to administer skilled nursing care for terminally ill patients of all ages in their place of residence, triage calls after business hours, and evaluate patients' appropriateness for Hospice services. Requirements

  • Current State license as a Registered Nurse
  • Current State Driver's License
  • Reliable transportation
  • Valid and current auto liability insurance
  • Two years experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred
  • Nursing skills as defined as generally accepted standards of practice
  • Good interpersonal skills
  • Specialized skills preferred in palliation and end-of-life
  • Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients
  • Requires hand-eye coordination and manual dexterity

Nice To Haves

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing

Responsibilities

  • Evaluate and assess patients' appropriateness for Hospice services
  • Carry out the admissions process of terminally ill patients
  • Respond to patient care visits to assess and treat patient, as needed
  • Triage incoming patient calls after business hours
  • Conduct routine Homecare visits at the patients' place of residence
  • Conduct pronouncement visits at the time of the death of the patient
  • Complete chart audits, as requested
  • Provide RN assistance at the Hospice of South Texas Inpatient Center, as needed
