About The Position

Tapestry Hospice & Palliative Care is a family-owned and operated organization that has been serving North Georgia for over 16 years. We are dedicated to providing compassionate care to our patients and their families, treating everyone like family. Your role as an On-Call RN will involve creating, planning, and directing hospice care, utilizing the nursing process to assess, intervene, implement, and evaluate patient care. You will collaborate effectively with the Interdisciplinary Team, patients, and their loved ones, maintaining high standards of care and professional competency. The On-Call RN provides holistic nursing care to terminally ill patients and their families outside of normal business hours, offering support during times of death. This care is grounded in the nursing process, including patient assessments, problem diagnosis, patient education, and outcome evaluation.

Requirements

Certified Registered Nurse.
Ability to work a Friday - Sunday schedule.
Excellent clinical assessment skills.
Ability to provide patient education and evaluate patient outcomes.
Time management skills.
Advanced knowledge of pain management & symptom control.

Nice To Haves

Hospice case management.
Critical Care, Surgery or Oncology experience.

Responsibilities

Create, plan and direct the hospice care team utilizing nursing processes of assessment, interventions, implementation and evaluation.
Effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.
Provide holistic nursing care to terminally ill patients and their families after normal business hours.
Provide appropriate support for families at time of death.
Perform admission evaluations and pronouncements.
Conducts follow-up visits.
