About The Position

Tapestry Hospice & Palliative Care is a family-owned and operated organization that has been serving North Georgia for over 16 years. We are dedicated to providing compassionate care to terminally ill patients and their families, treating everyone like family. We are seeking an exceptional On-Call RN to join our growing team. In this role, you will create, plan, and direct hospice care, utilizing the nursing process to provide holistic care. You will collaborate with the Interdisciplinary Team, patients, and their loved ones, maintaining the highest standards of care and clinical competency. The On-Call RN provides essential nursing care, support, and education to patients and families outside of normal business hours, including support at the time of death.

Requirements

Certified Registered Nurse
Ability to work a Friday - Sunday schedule
Excellent clinical assessment skills
Performs admission evaluations and pronouncements
Ability to provide patient education and evaluate patient outcomes
Time management skills
Advanced knowledge of pain management & symptom control

Nice To Haves

Hospice case management experience
Critical Care experience

Responsibilities

Create, plan, and direct the hospice care team utilizing nursing processes of assessment, interventions, implementation and evaluation.
Effectively interact with all members of the Interdisciplinary Team as well as with patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.
Provide holistic nursing care to terminally ill patients and their families after normal business hours.
Provide appropriate support for families at time of death.
Perform patient assessments, diagnose key problems, provide patient education and evaluate patient outcomes.
Conducts follow-up visits.
