Requirements

• Active RN license (Indiana)
• 1+ year clinical experience
• Ability to travel locally
• Valid driver’s license

Responsibilities

• Manage your own patient caseload
• Complete Start of Care and follow-up visits
• Develop and update care plans
• Communicate with physicians and care team
• Educate patients and families
• Document in Homecare Homebase
