This position includes access to a company fleet vehicle, helping you travel between patient visits without putting wear and tear on your personal car. What You Will Do • Provide skilled nursing care in patients’ homes under RN supervision • Follow physician orders and plan of care • Educate patients and caregivers on treatment and self-care • Monitor patient condition and report changes to the RN Case Manager • Complete accurate and timely clinical documentation • Promote patient safety and help prevent hospital readmissions • Build trusting relationships with patients and families What You Bring • Active Licensed Practical Nurse (LPN) license • Graduate of an accredited nursing program • 1 year of clinical experience preferred • Home Health or Hospice experience preferred • Ability to work independently in the field • Valid driver’s license and acceptable driving record • Ability to lift and assist patients as needed Curious About Home Health? Requirements

  • Active Licensed Practical Nurse (LPN) license
  • Graduate of an accredited nursing program
  • 1 year of clinical experience preferred
  • Home Health or Hospice experience preferred
  • Ability to work independently in the field
