About The Position

Puget Sound Hospice of King County is seeking a compassionate and resourceful Social Worker to join their mission-driven hospice team. This role focuses on providing quality end-of-life care, bringing comfort and dignity to patients and their families. As a Hospice Social Worker, you will help patients and families navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care in a home setting.

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited program
  • Current Washington State Social Work License (LICSW, LASW, LSWAIC,LSWAA)
  • Knowledge of community resources and end-of-life care planning
  • Strong communication and interpersonal skills
  • Comfortable traveling within Pierce and King County

Nice To Haves

  • Prior hospice or palliative care experience strongly preferred

Responsibilities

  • Conduct comprehensive psychosocial assessments and develop individualized care plans for hospice patients.
  • Provide clinical counseling and emotional support to patients and families, addressing social, emotional, behavioral, and spiritual concerns.
  • Educate patients and families about hospice services, end-of-life planning, advance directives, and community resources.
  • Offer guidance on behavioral health issues and provide crisis intervention when necessary.
  • Advocate for patient needs, ensuring their rights are respected and upheld within the healthcare system.
  • Collaborate with the interdisciplinary hospice team to coordinate holistic care and ensure continuity of services.
  • Maintain accurate, timely, and confidential documentation in compliance with HIPAA and organizational standards.
