About The Position

Puget Sound Hospice of King County is seeking a compassionate and resourceful Social Worker to join their mission-driven hospice team. This role focuses on providing quality end-of-life care, bringing comfort and dignity to patients and their families. As a Hospice Social Worker, you will help patients and families navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care in a home setting.

Requirements

Master’s Degree in Social Work (MSW) from an accredited program
Current Washington State Social Work License (LICSW, LASW, LSWAIC,LSWAA)
Knowledge of community resources and end-of-life care planning
Strong communication and interpersonal skills
Comfortable traveling within Pierce and King County

Nice To Haves

Prior hospice or palliative care experience strongly preferred

Responsibilities

Conduct comprehensive psychosocial assessments and develop individualized care plans for hospice patients.
Provide clinical counseling and emotional support to patients and families, addressing social, emotional, behavioral, and spiritual concerns.
Educate patients and families about hospice services, end-of-life planning, advance directives, and community resources.
Offer guidance on behavioral health issues and provide crisis intervention when necessary.
Advocate for patient needs, ensuring their rights are respected and upheld within the healthcare system.
Collaborate with the interdisciplinary hospice team to coordinate holistic care and ensure continuity of services.
Maintain accurate, timely, and confidential documentation in compliance with HIPAA and organizational standards.
