About the Role As a Hospice Social Worker, you will provide psychosocial support, counseling, education, advocacy, and resource coordination for hospice patients and their loved ones. Working alongside physicians, nurses, chaplains, aides, and other members of our interdisciplinary team, you will help patients navigate one of life's most important journeys with dignity and compassion. This position will primarily support patients throughout North Austin and surrounding communities. While territories generally follow geographic regions, we work collaboratively as a team and maintain flexibility to best serve our patients. As we continue to grow, you'll be joining an established social work department, partnering with an experienced Hospice Social Worker to ensure comprehensive support for the families we serve.

Requirements

  • Current Texas LMSW or LBSW license in good standing.
  • Bachelor's or Master's degree in Social Work (Master's preferred but not required).
  • Valid Texas driver's license, reliable transportation, and current auto liability insurance.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.

Nice To Haves

  • One or more years of hospice, palliative care, oncology, home health, or healthcare social work experience.
  • Knowledge of community resources throughout the Austin area.
  • Experience supporting patients and families through serious illness or end-of-life care.

Responsibilities

  • Complete comprehensive psychosocial assessments and develop individualized plans of care.
  • Provide emotional support, counseling, and crisis intervention for patients and families.
  • Assist with advance care planning, end-of-life planning, and community resource coordination.
  • Connect patients and caregivers with financial, social, and community resources.
  • Support patients and families through end-of-life transitions, grief, and bereavement.
  • Collaborate closely with the interdisciplinary hospice team to coordinate exceptional patient care.
  • Participate in care planning meetings and contribute to individualized care plans.
  • Maintain timely, accurate clinical documentation.
  • Participate in quality improvement initiatives and continuing education.
  • Promote the philosophy and mission of hospice through compassionate, patient-centered care.
