About The Position

Suncrest Hospice is seeking a full-time LPN to provide care and services for a group of hospice patients. This role involves visiting patients in their residences, including private homes, assisted living facilities, and hospitals. The LPN will be responsible for completing hospice admissions and patient visits as needed, answering calls from patients and families, and advocating for hospice services. The position requires a passion for hospice care and excellent clinical and communication skills.

Requirements

  • Current LPN licensure (in good standing) in the state(s) of practice.
  • One year of experience under license.
  • Great clinical and communication skills.
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.
  • Ability to learn an electronic medical records system.

Nice To Haves

  • Admissions and On-call experience preferred.
  • Works well within a team environment.
  • Can work independently and on your own from a day-to-day basis.
  • Understanding of hospice rules and regulations.

Responsibilities

  • Provide care and services for a group of hospice patients.
  • Visit patients where they reside (private homes, assisted living facilities, hospitals).
  • Complete hospice admissions and patient visits as needed.
  • Answer calls from patients and families.
  • Make visits as needed.
  • Triage calls.
  • Participate in making visits.
  • Address other on-call needs.
  • Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level.
  • Manage visits, documentation, and meetings.
  • Respond to urgent needs by prioritizing and triaging multiple concerns.
  • Learn an electronic medical records system.
