Weekend Hospice LPN
About The Position
Croí Health is a mission-driven, non-profit organization providing home health, palliative, hospice, and wellness services across 27 communities, from Milton to Plymouth. The Hospice Weekend LPN will work collaboratively with the RN case manager to provide skilled nursing services to patients in their homes. You are responsible for the direct patient care and to provide any changes in status directly to the physician and case manager. You must work collaboratively within the team structure and have strong communication skills.
Requirements
- Current Massachusetts Licensed Practical Nurse license in good standing.
- 2+ years of LPN experience required.
- 2+ years of Hospice experience required.
- Strong clinical assessment skills and the ability to prioritize and triage patients.
- Exceptional communication skills and the ability to work within a team structure.
- Ability to work independently and understanding of how to access resources for patients.
- Excellent computer skills.
Nice To Haves
- Prior homecare and hospice experience preferred.
