About The Position

This role serves as a Weekend Inpatient Hospice Nurse Liaison/Case Manager. The Home Health Nurse is responsible for providing coordinated, comprehensive nursing care to patients in the home setting. This involves utilizing critical thinking, analysis, and clinical judgment to address the physical, psychosocial, and spiritual needs of patients and families facing acute, chronic, and life-limiting illnesses, with the goal of improving function, independence, and quality of life. Requirements

• Current license to practice nursing in PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
• Valid Driver's license and automobile insurance.
• Provider BLS (American Heart Assoc/American Red Cross) required.
• Two Years recent med-surg experience in an acute or long-term care setting, rehabilitation, home care, or hospice setting preferred.

Nice To Haves

• BSN preferred.
• CHPN preferred.

Responsibilities

• Providing coordinated, comprehensive nursing care to patients in the home setting.
• Utilizing critical thinking, analysis, and clinical judgment in care delivery.
• Addressing the physical, psychosocial, and spiritual needs of patients and families.
• Improving patient function, independence, and quality of life.
• Participating in hospital & system-wide Nursing Committees, Unit Councils, evidence-based research projects, and precepting new staff.
