This role involves serving a caseload of clients, with an average of 38 clients per caseload, which is lower than the industry standard. Key responsibilities include completing new client intakes, developing initial service plans with clients and their legal representatives, coordinating services between providers and clients, scheduling in-person meetings with clients at least twice a year, and making monthly phone contact with each client. The position also requires maintaining quality documentation, such as case notes, service plans, and service agreements. The Case Manager will work with clients to identify their needs and aspirations, connect them to appropriate services and resources, and act as a liaison to ensure their care team is aligned in supporting their goals.
