About The Position

Keystone is seeking a Waiver Case Manager Supervisor to provide leadership, support, and supervision to Waiver Case Management staff. These staff members provide case management services for individuals enrolled in Home and Community Based Waiver Service Programs in Ramsey County and Hennepin County, specifically the Alternative Care (AC) and Elderly Waivers (EW) programs. The role involves assisting with employee hiring and training, intern management, planning and development of program services, reporting, and ensuring contract compliance. The supervisor will also maintain a small caseload and be responsible for delivering client services in accordance with Keystone standards.

Requirements

  • A bachelor's degree in Social Work or related field.
  • 2 years of Case Management experience including working knowledge of case management methods/values/principals of social work practice.
  • Understanding of professional boundaries, community resources, supervisory principles and interventions, program planning and development.
  • Familiarity with Federal and State Home and Community based service rules, regulations, and policies.
  • Cultural competency.
  • Knowledge of case management issues for those experiencing serious mental illness or developmental disability.
  • Understanding of mental health and social service rules, regulations, and policies.
  • Experience with billable time/units.
  • A valid Minnesota Driver's License and insurance.
  • Ability to travel within Ramsey County and surrounding areas for home visits.

Nice To Haves

  • Social Work Licensure with the Minnesota Board of Social Work, with ability to provide licensure supervision.
  • An advanced degree in Social Work or related field.
  • Previous experience in Home and Community Based Services/Waiver programs.
  • Prior Supervisory and training experience including strong leadership and coaching skills.

Responsibilities

  • Supervise and manage a team of Disability Services case managers.
  • Monitor staff performance and productivity, providing feedback, coaching, and discipline as needed.
  • Screen, interview, and hire case managers.
  • Provide regular case consultations and serve as a resource for case managers.
  • Teach best practices and optimum customer service techniques.
  • Conduct file reviews and maintain caseload review spreadsheets.
  • Provide leadership and direction on policy and procedures for compliance.
  • Train and orient new staff, and provide ongoing training for current staff.
  • Organize and lead regular team meetings.
  • Provide regular feedback and annual performance reviews for staff.
  • Authorize time off and timesheets for staff.
  • Review and assign new referrals from Ramsey County to case managers.
  • Maintain and update training materials.
  • Stay updated on changes in programs, policies, and compliance requirements.
  • Evaluate program requirements and develop/maintain processes and procedures for program quality.
  • Coordinate with supervisors at Ramsey County and submit required reports.
  • Review and approve requests for individual client budget amendments.
  • Prepare and maintain reports.
  • Work collaboratively with internal and external partners.
  • Provide backup for staff and Program Director as needed.
  • Provide person-centered case management services to individuals receiving Home and Community Based Waiver Services.
  • Develop and implement person-centered Coordinated Services and Support Plans (CSSPs).
  • Evaluate, facilitate, and monitor services and individual progress.
  • Plan and arrange for services via Service Agreement and inform Ramsey County Clerk of proposed services.
  • Act as a liaison with community and referral services.
  • Maintain necessary records for continuity of service and reports to funding agencies.
  • Maintain accurate case notes, documenting all actions related to case management coordination.
  • Complete all required paperwork, reports, and charting simultaneously and in an organized manner.
  • Participate in team meetings, agency all staff meetings, health plan meetings, and trainings.
  • Perform other duties or projects as assigned.
  • Obtain necessary CEUs to maintain professional licensure status if applicable.
