Requirements

  • Active RN licensure in the state of employment; MSN preferred
  • 5+ years of hospice leadership experience required
  • Certified Hospice & Palliative Care RN (CHPN) preferred
  • Excellent knowledge of hospice regulations, CHAP standards of practice, and state guidelines
  • Strong understanding of performance improvement, quality assessment, and utilization management
  • Demonstrated ability to develop clinical and non-clinical staff across multiple locations
  • Proven ability to work effectively in an interdisciplinary, matrixed leadership environment
  • Proficiency in Microsoft Word, Excel, and Google Workspace
  • Self-directed with the ability to lead with minimal supervision across a multi-state footprint
  • Deep commitment to the palliative philosophy and the principles of end-of-life care

Responsibilities

  • Directing and administering nursing and patient care services to ensure care meets current medical and nursing standards
  • Maintaining ongoing quality assessment, performance improvement analysis, and evaluation of patient care delivery
  • Providing oversight of occurrences, complaints, negative trends, patient/family satisfaction scores, and performance improvement issues
  • Ensuring compliance with CHAP, CMS, and state guidelines across all service lines and locations
  • Assuring competency of all personnel in alignment with state, federal, and CHAP requirements
  • Mentoring Administrators on fiscal, clinical, census, and quality metrics for their assigned agencies
  • Supporting the employee selection process, work assignments, performance evaluations, and staff development across nursing and patient care services
  • Reviewing and approving people decisions — including new hires, terminations, talent reviews, and grievances — in partnership with Human Resources
  • Recommending and participating in education, training, and orientation programs; actively encouraging professional advancement of staff
  • Working collaboratively with senior leadership to ensure goals are actualized and staff reach their highest potential
  • Partnering with the CFO and nursing leadership to plan budgeting requirements for personnel, supplies, and equipment
  • Maintaining cost controls to ensure maximum effectiveness of approved departmental budgets
  • Planning and recommending new facilities, equipment, or modifications needed to support patient care
  • Recommending modifications to personnel policies to ensure appropriate working conditions and care coverage
  • Maintaining and advancing the Transitions culture across all service locations
  • Initiating and participating in problem-solving and policy-forming conferences for nursing and patient care services
  • Interpreting the goals of patient care services to the community through relationships with education systems, volunteer groups, and partner agencies
  • Maintaining close coordination across all departments to ensure continuity and collaboration of services
