About The Position

Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with agencies in Peel Region to offer services at the new Brampton-based Homeless and Addictions Recovery Treatment (HART) Hub. The HART Hubs are community-based care hubs designed to address mental health, addiction, and housing needs for individuals experiencing homelessness. They provide integrated care, including primary care, mental health services, addiction care, and employment supports. Through targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them achieve recovery, housing stability, and inclusion.

Requirements

  • Post-secondary degree or diploma in a relevant discipline.
  • Minimum 2 years of experience in vocational rehabilitation, career counselling, and/or adult education with individuals experiencing complex barriers to employment.
  • Empathize and connect with those experiencing the mental health and addictions system.
  • Demonstrated experience working within a multidisciplinary team and evidence of an ability to collaborate and partner.
  • Working knowledge and/or an understanding of various disciplines; including but not exclusive of mental health, substance use, harm reduction, newcomer challenges and barriers faced by those in marginalized populations.
  • Demonstrated cultural competence, ability to work and apply practical understanding of different elements of diversity, equity, inclusion and belonging as well as an understanding of social determinants of health and impact on vulnerable communities.
  • Knowledge of a variety of evidence-informed treatment modalities including motivational interviewing and trauma informed practices.
  • Experienced at engaging in matters that require sensitivity and empathy across a diverse community, excellent judgment, and discretion; ability to operate with various frameworks including culturally sensitive, anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional.
  • Valid Canadian driver’s license and access to an insured automobile in good repair.
  • A satisfactory Vulnerable Sector Screening (Police Check) report.

Nice To Haves

  • Registration with a regulatory college is considered an asset.
  • Knowledge and/or experience working with the following systems is an asset: ODSP, Ontario Works, CPP, Disability insurance provider.

Responsibilities

  • Conduct comprehensive standardized vocational assessments to develop employment-related goals and address barriers to employment.
  • Utilize tools such as aptitude assessments to assist clients in creating individual vocational plans.
  • Provide individual employment counselling for job readiness skills, interview preparation, job search techniques, resume development, and career exploration.
  • Support clients in developing empowerment skills and combating stigma through self-advocacy.
  • Manage a caseload including regular re-assessments and caseload reviews.
  • Maintain up-to-date knowledge of employment and labour market trends, including job openings and skill requirements.
  • Provide education to clients and employers about mental health in the workplace.
  • Collaborate with clients and the multi-disciplinary team to establish Recovery Plans which guide mutual work towards client recovery.
  • Work with prospective employers to identify and develop employment opportunities for clients.
  • Regularly assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate.
  • Offer drop-in support for computer skills, job seeking, and employment related activities in the Career Centre.
  • Facilitate recovery employment-related groups as needed, including developing new group materials.
