About The Position

Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with various agencies in the Peel Region, including SHIP, to offer services at the new Brampton-based Homelessness and Recovery Treatment (HART) Hub. HART Hubs are community-based care hubs designed to address mental health, addiction, and housing needs for individuals experiencing homelessness. They provide integrated care, including primary care, mental health services, addiction care, and employment supports. Through targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them achieve recovery, housing stability, and inclusion. CMHA Peel Dufferin is seeking passionate individuals to drive change and deliver evidence-based, responsive, and affirming care.

Requirements

  • Post-secondary education in Social Work, Health Services, Psychology or equivalent.
  • 3 years providing Mental Health case management or equivalent care.
  • Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment.
  • Experience with using addictions and concurrent disorders assessment tools.
  • Experience working within the homelessness sector.
  • Knowledge of evidence-based practice for mental health services.
  • Excellent assessment, risk management, and crisis intervention skills.
  • Expertise in collaboration with healthcare services.
  • Knowledge of MS Suite (Word, Excel, Power Point and Outlook).
  • A vehicle, valid driver’s license, and adequate insurance coverage.
  • Valid Certification in First Aid & CPR.
  • Training in ASIST, CPI and Naloxone administration.

  • Working knowledge and/or an understanding of various disciplines; including but not exclusive of mental health, substance use, harm reduction, newcomer challenges and barriers faced by those in marginalized populations.
  • Demonstrated cultural competence, ability to work and apply practical understanding of different elements of diversity, equity, inclusion and belonging as well as an understanding of social determinants of health and impact on vulnerable communities.
  • Knowledge of a variety of evidence-informed treatment modalities including motivational interviewing and trauma informed practices.
  • Experienced at engaging in matters that require sensitivity and empathy across a diverse community, excellent judgement, and discretion; ability to operate with various frameworks including culturally sensitive, anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional.

Responsibilities

  • Liaise with the intake and HART Hub team to support successful transition into supportive housing using a housing first approach.
  • Support clients’ transition into supportive housing and promote housing stability.
  • Develop and implement individualized wellness, crisis, and recovery plans.
  • Conduct comprehensive standardized assessments to enhance clients’ mental and physical health and community integration.
  • Provide strengths-based case management and assist with developing daily skills.
  • Facilitate access to holistic, person-centred services addressing social determinants of health.
  • Regularly assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate.
  • Partner with clients, families, and other health care team members.
  • Facilitate recovery and/or social-recreational groups as needed, including developing new group materials.
  • Maintain a network of community resources and coordinate referrals.
  • Work collaboratively and independently within a multidisciplinary team.
  • Maintain accurate records and statistical data in line with agency standards.
  • Update all appropriate documentation within 24 hours of client contact.
  • Actively participate in staff meetings, team huddles, supervisions, case conferences, trainings, and events as required.
  • Uphold agency health and safety and client safety protocols.
  • Promote inclusive, equitable, anti-racist, and anti-oppressive practices.
  • Comply with PHIPA legislation and demonstrate strong communication and organizational skills.
  • Perform additional duties as required/delegated by Management.
