About The Position

Community Living, Inc. is looking for a Vocational Case Manager to help individuals with intellectual and developmental disabilities discover meaningful employment opportunities. In this role, you'll work one-on-one with participants to identify their strengths, connect with local employers, and provide the support they need to succeed in the workplace. Every day is different. One day you might be meeting with a local business to develop a new employment partnership, and the next you could be helping a participant prepare for an interview or coaching them through their first days on the job. Requirements

  • High school diploma or GED required.
  • Strong communication, organization, and time management skills.
  • Comfortable balancing multiple participants and priorities.
  • Ability to build positive relationships with employers and community partners.
  • Proficiency with Microsoft Office.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Bachelor's degree in Social Services or a related field preferred.
  • Experience supporting individuals with intellectual and/or developmental disabilities is preferred.

Responsibilities

  • Partner with participants to identify employment goals and career interests.
  • Build relationships with local employers and develop job opportunities.
  • Conduct assessments to match participants with the right employment.
  • Assist participants with job searches, applications, and interviews.
  • Provide job coaching and on-site support to help participants succeed.
  • Monitor employment progress and adjust supports as needed.
  • Complete documentation, case management, and monthly reporting requirements.
  • Coordinate individual plans and maintain participant records in Therap.
  • Collaborate with families, employers, and interdisciplinary teams.
  • Ensure services meet all funding and compliance requirements.
