Bayshore is currently seeking a passionate and caring Visiting Nurse to join their exceptional care team in the community. This role involves home-to-home visits in the Greater Hamilton and Burlington Area, requiring driving within the community. The position offers a pay-per-visit structure with shifts from Monday through Friday, alternating weekend rotations, and varying shift times. An on-call and statutory holiday rotation is also part of the role.

Requirements

  • Graduate of an accredited School of Nursing (RN)
  • Current registration in good standing with the provincial regulatory body (CNO)
  • 2-5 years of recent relevant nursing experience in a fast-paced clinic or medical environment
  • Valid G2 or G Driver’s License and access to a reliable vehicle
  • Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions

  • Experience with wound care, diabetes management, trach/vent, IV care is an asset

Responsibilities

  • Participate in on-call rotations as required (typically twice per month overnights)
  • Conduct in-home assessments and deliver nursing care based on individualized care plans
  • Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
  • Monitor patient progress and update care plans accordingly
  • Educate patients and caregivers on disease management, medication adherence, and health promotion.
  • Collaborate with interdisciplinary teams and task delegation to ensure continuity of care
  • Maintain accurate and timely documentation using electronic health records
  • Teach, delegate tasks and functions to different levels of clients and family members while maintaining the industry’s highest level of practice.
