The Vision Care Coordinator provides onsite support for the coordination of vision health care in the DoD Military Health System and the VA Health System. This role involves supporting individuals with vision-threatening conditions such as ocular trauma and other vision loss injuries, ensuring timely and appropriate care by coordinating vision care specialists and patient movement through the health system. The position also supports beneficiaries with high-risk conditions like retinopathy of prematurity and involves creating a nationwide coordination network with military hospitals, clinics, and VA facilities across the United States and overseas for ocular care management. The coordinator will arrange eye care for patients, including managing referrals, patient movement, follow-ups, and monitoring performance metrics to facilitate transfers between Military Treatment Facilities (MTF) and allied healthcare providers like the VA health system. Travel within the Continental United States (CONUS) and the National Capitol Region (NCR) is required for meetings, conferences, and training.
