About The Position

Ennoble Care is a mobile primary care, palliative care, and hospice service provider seeking a Virtual Supervising Physician to provide oversight for approximately 3,000 homebound patients. This role aims to bridge the gap in physician involvement for patients receiving care from Advanced Practice Providers in their homes, ensuring annual physician visits, physician-reviewed care plans, and consistent oversight of care management. The physician will be responsible for ensuring patients receive the right care, from the right clinician, at the right time.

Requirements

  • MD or DO, board certified and maintained throughout employment (Internal Medicine, Family Medicine, or Geriatrics preferred).
  • Active, unrestricted license to practice medicine in MD/VA.
  • Willingness to obtain licensure in additional states served by Ennoble Care, at company expense.
  • Current DEA registration in good standing.
  • Eligible to enroll and bill as a Medicare practitioner; not excluded or debarred from any federal health care program.
  • Insurable under Ennoble Care's professional liability policy.
  • Able to work full-time and exclusively for Ennoble Care.
  • Commitment to a one-year term.
  • Comfortable with a high-volume telehealth schedule and productivity measurement.
  • Strong, efficient clinical documentation skills.

Nice To Haves

  • Home-based primary care, house-call, or PACE experience.
  • Experience supervising or collaborating with nurse practitioners and physician assistants.
  • Familiarity with Chronic Care Management, Advanced Primary Care Management, RPM, or BHI.
  • Experience in value-based care, ACO, MSSP, or Medicare Advantage.
  • Licensure already held in more than one of Ennoble Care's states, or Interstate Medical Licensure Compact eligibility.
  • Palliative or hospice background.
  • Spanish or another second language proficiency.

Responsibilities

  • Conduct approximately 20 telehealth visits per day (target 4,000–5,000 per year) with established homebound Medicare patients, utilizing video-first with audio-only options.
  • Review and sign the care plan for every patient on the panel annually.
  • Serve as the general supervising and collaborating physician for a pod of nurse practitioners and physician assistants, including executing and maintaining state collaboration agreements.
  • Review clinical escalations, conduct chart audits, and sign skills exams and operational checklists for clinical staff.
  • Train care coordinators on care management protocols and documentation standards.
  • Provide ongoing clinical guidance and quality-assurance feedback to APPs.
  • Complete ancillary services as add-ons during visits, such as Annual Wellness Visits, Advance Care Planning, cognitive assessments, and transitional care management.
  • Work entirely within Ennoble Care's purpose-built clinical and operations platform, OA.
  • Report to the Regional Chief Medical Officer for the market.
